At Swift Care Solutions, it is our aim is to provide a person-centred domiciliary care service to clients and their families who wish to remain in their own home. Provided by caring, trained, reliable Homecare Assistants.

We achieve this by involving both clients and their families in the planning of the care that will be needed following a face-to-face assessment at home; care home or hospital setting prior to commencement of the care package, thereby ensuring we can meet the client’s needs and expectations.

We regularly review the agreed Care Plan involving our client and their family on an ongoing basis and as needs change. We stay in touch through monthly telephone well-being calls, monthly care plan reviews, and home visits by a Care Home Manager/Care Co-Ordinator on a regular basis.

It is important to us to be able to enable and support people who use our service to retain and where possible regain their independence, and we will work with other healthcare services and resources as needed to ensure that this happens as much as possible.
